The evidence-based guideline "Improving Medication Management Older Adult Clients" (Bergman-Evans, 2004) provides assessments interventions that are useful across settings. By focusing on reducing inappropriate prescribing, decreasing polypharmacy, avoiding adverse events, maintaining function, opportunity improve outcomes this population.
