Is overactivity the core feature of hypomania in bipolar II disorder

摘要: Background: Recent studies found that overactivity (increased goal-directed activities) may be as important mood change (elevated and/or irritable mood) for the diagnosis of mania/hypomania (on family history and psychometric grounds), questioning DSM-IV-TR criteria always requiring listing among other symptoms. The aim study was to find out if at least hypomania. Sampling Methods: A consecutive sample 137 bipolar II disorder (BP-II) 76 major depressive remitted outpatients were interviewed with Structured Clinical Interview DSM-IV by a senior clinical research psychiatrist in private practice. Patients asked they had hypomanic symptoms episodes, which most common during various episodes. not been planned when variables collected different goals. Results: Overactivity symptom BP-II, more than elevated mood, strongest association BP-II all (overactivity odds ratio = 15.4, 12.6). Three factors found: an ‘elevated mood’ factor including increased self-esteem; ‘mental activation’ racing/crowded thoughts, ‘behavioral overactivity. There no relationship between change. Irritable associated mood. present 21.6%of patients without overactivity, 81.0% 25.0% 63.3% As predictor sensitivity 90.5%, specificity 61.8%, positive predictive value mood: 72.2, 82.8, 88.3%, respectively). Five or balanced combination (82.4%) (85.5%) 91.1%. 89.5% ≧5 symptoms, while 76.6%. Conclusions: Theresults seem support view core feature hypomania, suggesting upgrading stem criterion
