摘要: The data generated in the process of medical care has historically not just been underused, it wasted. This was due part to difficulty accessing, organizing, and using entered on paper charts, but notable variability clinical documentation methods quality made problem even more challenging. In absence a practical way systematically capture, analyze, integrate information contained massive amount during patient care, medicine remained highly empirical which disconnected application individual experiences subjective preferences continues thwart continuous improvement consistent delivery best practices all patients.
