TMJ response to mandibular advancement surgery: an overview of risk factors

摘要: Objective: In order to understand the conflicting information on temporomandibular joint (TMJ) pathophysiologic responses after mandibular advancement surgery, an overview of literature was proposed with a focus certain risk factors. Methods: A search carried out in Cochrane, PubMed, Scopus and Web Science databases period from January 1980 through March 2013. Various combinations keywords related TMJ changes [disc displacement, arthralgia, condylar resorption (CR)] aspects surgical intervention (fixation technique, amount advancement) were used. hand these papers also identify additional articles. Results: total 148 articles considered for this and, although methodological troubles common, review identified relevant findings which practitioner can take into consideration during treatment planning: 1- Surgery unable influence preexisting displaced disc crepitus; 2- Clicking arthralgia not predictable there greater likelihood improvement rather than deterioration; 3- The counterclockwise rotation, rigidity fixation technique seemed position health; 4- CR increased, especially high-risk cases. Conclusions: Young adult females retrognathism increased plane angle are susceptible painful TMJ, subject less surgery prone CR. Furthermore, thorough evidenced-based studies required response surgery.
