Cadaveric renal transplantation using kidneys from donors greater than 60 years old.

摘要: Transplantation of kidneys from donors over the age 60 yr is controversial. However, as demand for cadaveric far exceeds supply, exploration usefulness outside currently accepted donor pool necessary. Between January 1987 and July 1989, 31 (5.5%) 558 renal transplants performed at University Pittsburgh utilized organs older than yr. Median recipient was 41 (range 24-71 yr); 4 recipients were diabetic 6 had panel-reactive antibody levels greater 20% time transplant. All treated with cyclosporine, prednisone azathioprine. The 1-yr allograft survival 65% which less but not statistically different graft 80% in a retrospective selected control group who received grafts younger aged 11 to 50 cold ischemia 48 hours 38%, significantly poorer 78% seen times h (p=0.04 Breslow). mean serum creatinine higher 1, 3, 12 months post-transplant even when excluded. In summary, transplantation cadaver results acceptable rates. These are more susceptible ischemic injury function donors. Expansion by use could have an impact on alleviating chronic national kidney shortage.
