摘要: Aims For many young people with Type 1 diabetes, transition from paediatric to adult care can result in a marked deterioration glycaemic control. A systematic review assessed the effectiveness of models, or components for managing process long-term conditions, including diabetes. This involved identifying (i) main barriers and facilitators implementing successful programme, (ii) key issues conditions professionals process. Methods The following databases were searched inception August 2012: MEDLINE, EMBASE, PsychINFO, CINAHL, ASSIA, Social Services Abstracts, Academic Search Complete, Science Citation Index, Cochrane Campbell Libraries. Selected studies included aged 11 25 diagnosed who secondary health services. Results 16 reviews 13 primary 9992 records retrieved. No single model was uniquely effective. most transitions centred around: person-focused; age developmentally appropriate content delivery; self-management education; family participation; collaboration; designated clinics; co-ordinator; person’s portfolio; specific training; multidisciplinary approach; structured embedded service delivery.
