The International Perspective on Healthcare Transition

摘要: This chapter identifies effective models and components of good practice in supporting young people as they move from children’s to adults’ health services, an international perspective. The builds on systematic review evidence identify recent lessons emerging practice. There is insufficient one single model best overall. is, however, consensus the literature about broad principles that should underpin all some promising clusters which others can learn.
