Treatment of Malignant Gliomas in the Elderly

摘要: The benefit of standard treatment malignant glioma in older patients is debated. In order to assess the effect a combination surgery, radiotherapy and chemotherapy on survival elderly with high grade gliomas, 30 consecutive than 70 years supratentorial gliomas were studied between 9/93 9/96. Median age was 73 (70–79). mean Karnofsky performance status (KPS) 66 (30–100). Patients underwent maximum possible followed by course (45 Gy/25 fractions/5 weeks) 3 or 4 orthogonal beams 2 cm margin around tumor bed. administration left at discretion responsible physician 12 received reduced dose nitrosourea-based chemotherapy. overall median 36 weeks. time progression 26 Three months after alive, 5 complete response, partial response 10 stabilized. Pre-radiotherapy KPS only significant prognostic factor 40 weeks ≥70 25 when >70 (logrank test, p=0.05). responding stable (57% group) 68 1 completion radiotherapy. There no case radiotherapy-induced dementia this regimen. Four out who chemotherapy, experienced WHO 3/4 hematotoxicity.
