The role of bronchoalveolar lavage in the diagnosis of invasive pulmonary aspergillosis.

摘要: Cultures and histochemical stains for fungi were performed on concentrated, cytocentrifuged bronchoalveolar lavage (BAL) samples from 82 immunocompromised patients undergoing bronchoscopic evaluation of new pulmonary infiltrates. Aspergillus hyphae identified in 9 17 BAL with invasive aspergillosis 3 the remaining 65 study without this diagnosis. Thus, presence had a 53% sensitivity, 97% specificity, 75% positive predictive value diagnosis aspergillosis. fungal cultures only 4 cases (23% sensitivity). A combination yielded diagnostic sensitivity 58% specificity 92%. Results routine fiberoptic bronchoscopy procedures who retrospectively group 10 tabulated. Routine 5 27 (22% overall yield), washings (15%), transbronchial biopsies histologically 2 11 (18%). is valuable first procedure diagnosing compromised host.
