Relationship between time of day, day of the week and in-hospital mortality in patients undergoing emergency percutaneous coronary intervention.

摘要: Contexte. - De precedentes etudes ont montre des variations circadiennes dans les taux de mortalite et complications post-angioplastie. Objectifs. — Etudier si Les evenements survenus au cours 48 premieres heures apres l'admission a l'hopital sont en relation avec le moment jour ou l'angioplastie ete realisee urgence. Methodes. angioplasties realisees urgence (n = 2266 total, 1396 ouvrables 870en non ouvrables) sur une periode 3,5ans (2005-2008) analysees. Le critere primaire etait la secondaire un combine d'evenements cardiovasculaires. L'analyse statistique par regression logistique multivariee score propension. Resul tats. L'âge moyen 64,8 ans 77,3 % etaient hommes. 3,6 pour patients traites angioplastie durant nuit pic 5,1 ceux entre minuit 03 h 59, 3% du week-end compares 1,5% ouvrables. La frequence survenue autres cliniques n'a pas varie fonction jour. Compare aux semaine, risque relatif est 2,95 (95% IC 1,58-6,01 ; p=0,0007) pendant 2,42 0,97-6,01 p=0,06) week-end. Conclusion. L'etude effet temps-dependant significatif deces intrahospitaliers chez L'organisation soins ischemiques pourraient expliquer ces differences.
