摘要: Background The American Joint Committee on Cancer 8th edition staging manual for adrenocortical carcinoma (ACC) redefines T stage to include large vessel invasion (T4, previously undescribed) and restricts IV those with distant metastases. We evaluated the prognostic power of edition. Methods Patients ACC treated between January 1, 2000, December 31, 2015, were identified. Overall survival (OS) was compared using Kaplan-Meier Cox proportional hazard models. Results Of 290 patients evaluated, change in nomenclature impacted 13 (4.5%) who categorized as T3; 61 had involvement but already T4 based adjacent organs. restriction metastases downstaged 41 (14.1%; T4N0M0 or T3-4N1M0) III. In 7th edition, ratio (HR) death similar II III disease, 5-year OS 66.7%, 54.4%, 57.2%, 14.0% (stages I, II, III, IV, respectively). stages I remain unchanged, 44.1% 9.2%, respectively. c-index editions (83.4 82.7, Conclusions While changes impact a relatively small proportion patients, they represent progress toward common system that accurately reflects prognosis. inclusion tumors nodal disease rather than results improved stratification
