摘要: Resume L’histoire naturelle des infections osteo-articulaires (IOA) de l’enfant a ete considerablement modifiee par les progres l’antibiotherapie. Meme si depuis 10 ans la prise en charge pluridisciplinaire tend se simplifier, le traitement ideal osteomyelites aigues (OMA) et arthrites septiques (AS) chez l’enfant, particulier termes choix l’antibiotique, duree voie d’administration, reste relativement mal defini. Ce travail pour objectif principal d’evaluer un protocole IOA qui avait mis place 1993 au CHU Rouen, comparaison l’etude 2 series d’un total 210 patients, d’en tirer enseignements d’insuffisances afin d’elaborer nouvelles recommandations. Ainsi, apres mise du protocole, significatifs ont realises : certitude diagnostique est passee OMA 67,6 % 85,7 grâce une realisation plus systematique bilan ; documentation bacteriologique 37,5 72,2 41,6 59,5 AS, avec notamment reconnaissance croissante Kingella kingae (4 AS OMA) aux techniques prelevement culture durees moyennes intraveineux (15 jours versus 9,95 jours), totales (47,3 J vs 33,8 J) ainsi que d’hospitalisation (17,5 12,5 significativement diminuees. A l’issue ce travail, fonction insuffisances relevees, particularites epidemiologiques relevees donnees recentes litterature, reactualise simplifie.
