Efficacy of arthroscopic lysis and lavage in patients with chronic locking of the temporomandibular joint

摘要: This prospective 1-year follow-up investigated the efficacy of temporomandibular joint lysis and lavage in relation to arthroscopic diagnoses. The study comprised 42 patients. Clinical evaluation was performed before surgery at regular intervals up 1 year postoperatively. Diagnostic arthroscopy immediately subsequent were under local anesthesia all Only unilateral upper compartment arthroscopies executed. results evaluated according 1984 criteria American Association Oral Maxillofacial Surgeons. Osteoarthrosis diagnosed arthroscopically 30 joints (71%), 21 which (70%) had more pronounced changes. Synovitis, mainly a mild, localized type, 39 (93%). More synovitis found 12 (31%). Adhesions 20 (48%) significantly correlated with displaying osteoarthrosis (P < 0.001). overall success rate for 50% (21/42 joints). best response diagnosis (8/12 joints; 67%). All regarded as unsuccessful operated diskectomy without implants. 18 (86%).
