摘要: The concept of magnetic resonance perfusion-diffusion mismatch (PDM) provides a practical and approximate measure the tissue at risk has been increasingly applied for evaluation hyperacute acute stroke in animals patients. Recent studies demonstrated that PDM does not optimally define ischemic penumbra; because early abnormality on diffusion-weighted imaging overestimates infarct core by including part penumbra, perfusion weighted penumbra regions benign oligemia. To overcome these limitations, many efforts have made to optimize conventional PDM. Various alternatives beyond are under investigation order better penumbra. theory least three purposes: be used as selection tool treatment; test hypothesis patients with pattern will benefit from treatment, while those without not; surrogate outcome. main patterns its relation clinical outcomes were also briefly reviewed. conclusion was documented more reperfusion, reduced growth compared PDM, but it yet clear thrombolytic therapy is beneficial when selected Studies based larger cohort currently further validate hypothesis.
