摘要: Objective To describe the features of undifferentiated arthritis in an early synovitis out-patient clinic. Methods In a two-year prospective cohort study 320 patients with rheumatic symptoms less than one year were investigated Besides clinical parameters, intensive laboratory program was performed, including routine blood and serum immunological investigations (CRP, IgG,A,M,C3C4,RF,ANA, DNA, HLA B-27), microbiological to search for reactive arthritis-inducing infections. Results 217 had inflammatory diseases, whom only 100 (46%) could be given definite diagnosis, whereas 117 (54%) considered as having (UA). Patients UA mean age 41 +/- 15 years, sex-ratio 1.8:1.0 (f/m), joint manifestations oligoarticular 68%, monarticular 14%, polyarticular 18%. Rheumatoid factor positive 17%, HLA-B27 found 27%, 21% history recent infection. Follow up over 26 (range 4-38) months 28 (24%) revealed complete remission (54%), while 10 (36%) further partial remission, unchanged activity or progressive disease, 2 (7%) developed rheumatoid (RA) 1 (4%) diagnosed ankylosing spondylitis. Conclusion Most followed at our clinic remain unclassified good prognosis.
