Computational probabilistic quantification of pro-arrhythmic risk from scar and left-to-right heterogeneity in the human ventricles

摘要: Both scar and left-to-right ventricular (LV/RV) differences in repolarization properties have been implicated as risk factors for lethal arrhythmias. As a possible mechanism the initiation of re-entry, recent study has indicated that LV/RV heterogeneities adaptation action potential duration (APD) to changes heart rate can cause transient increase APD dispersion following acceleration, promoting unidirectional block conduction at junction. In presence an ischemic region ectopic stimulation, pathological suggested electrical re-entry. However, exact location timing activation play crucial role certain combinations may lead re-entry even under normal repolarization. This suggests phenomenon needs be investigated probabilistic way. this we employ computationally efficient, phenomenological model quantify pro-arrhythmic effects associated with range ofposition activation. allows us probabilistically how increasing interventricular increases arrhythmic risk. Results indicate larger more than doubles length time window during which ventricles are vulnerable reentry, leads four-fold probability within window.
