摘要: Surveillance bronchoscopies with transbronchial lung biopsies (TBBx) are often performed post-lung transplantation, but the clinical value and impact on outcomes remain controversial. Given cost risks associated TBBx, some centers only perform bronchoscopy for specific indications or events. Although presence of histological features (especially acute cellular rejection lymphocytic bronchiolitis) have been higher risk chronic allograft rejection, routine use mandated TBBx has not shown to alter outcomes. However, prospective, randomized studies done. Further, there limited published data regarding performing follow-up ensure resolution prior On other hand, putative benefits include ability fine tune immune suppression detect infection large airway stenoses that may require more aggressive measures minimize development strictures thereby prevent downstream post obstructive bronchiectasis. This review discusses technique criteria (acute chronic), complications (particularly pneumothoraces, hypoxemia, bleeding), surveillance in this complex patient population.
