摘要: Objective. Current guidelines of the Accreditation Council for Graduate Medical Education have restricted amount intensive care experience obtained during pediatric residency. The impact on performing procedures has not been evaluated. To determine current level competency in 1 common procedure, we investigated proficiency residents neonatal endotracheal intubation academic years 1998–1999 and 2000–2001. Methods. Indication intubation, number attempts, achievement success were recorded by respiratory therapist present procedure. Each was scored according to attempt which successful. Indications categorized as failure, delivery room resuscitation, meconium-stained amniotic fluid. Competency defined a successful occurring first or second ≥80% time. Intubation scores compared between at various stages training analyzed multivariate logistic regression analysis significance. Comparisons then performed percentage with confidence intervals. We also surveyed previous graduates program included observations this study asked them indicate how frequently they perform practice assess their own competence Results. A total 449 resident observed periods: 192 postgraduate year (PGY-1) residents, 126 PGY-2 131 PGY-3 residents. 35% (160 449) never house officers. 50% PGY-1 (95% interval [CI]: 42.6–56.8), 55% CI: 46–63.5), 62% 53.9–70.7). third-year exhibited significantly higher likelihood first-year showed no improvement third Surveys sent 56 our residency (1998–2000). Completed surveys received from 31 (66%) 47. 71% respondents are practicing general pediatrics, 36% attend deliveries intubations. 87% reported that good excellent after completion training. Conclusions. provide objective subjective data concerning intubation. None groups met specified definition technical competence, although there advancing bivariate analyses. However, felt confident skills contrast findings. As exposure these important becomes limited, methods ensure attainment may need be redefined.
