Prospective evaluation of contrast-enhanced MRI in the depiction of peritoneal spread in primary or recurrent ovarian cancer.

摘要: The purpose of this study was to assess the accuracy MRI in staging intra-abdominal tumor seeding ovarian carcinoma. Fifty-seven patients with suspected primary or recurrent carcinoma were included study. All received laparotomy within 8 weeks after MRI. protocol fat-saturated T1-weighted spin-echo (SE) sequences pre- and post i.v. application gadopentetate dimeglumine. criteria for manifestation contrast enhancement soft tissue lesions peritoneum. image review performed by a blinded radiologist, specialist gynecological abdominal MR imaging. Results compared findings during laparotomy. Sensitivity, specificity, positive negative predictive values, corresponding 95% confidence intervals calculated single locations groups defined according surgical strategies cytoreduction. Laparotomy histopathology confirmed 251 locations. Sensitivity high lower pelvis (73-83%) except ureter bladder infiltration (40%). Bowel mesentery displayed sensitivity values from 73 77%. very low pelvic lymph nodes (28%), greater omentum (38%), lesser sac (43%).
