摘要: Occipitoatlantoaxial rotatory fixation (OAARF) is a rare condition involving fixed rotational subluxation of the atlas in relation to both occiput and axis. Atlantoaxial (AARF) appears precede OAARF most cases, as untreated AARF may cause compensatory counter-rotation occipitoaxial at an apparently neutral head position. We report case 8-year-old girl with juvenile idiopathic arthritis. Cervical imaging demonstrated slight rightward rotation 7.63° C-2 significant C-1 65.90° 73.53° C-2. An attempt closed reduction halo traction was unsuccessful. Definitive treatment included open reduction, laminectomy, occipitocervical internal fusion.
