Clinical, Hormonal, and Neuroradiological Characteristics and Therapeutic Outcomes of Prolactinomas in Children and Adolescents at a Single Center.

摘要: Background/Purpose: A prolactinoma is the most common pituitary adenoma, but it relatively rare in childhood and adolescence. There only limited research about clinical spectrum, treatment, outcomes of prolactinomas In this single-center cohort study, we assessed clinical, hormonal, neuroradiological characteristics therapeutic children adolescents with prolactinomas. Methods: This retrospective study included 25 patients diagnosed before 19 years age, who presented at Samsung Medical Center during a 15-year period (March 2005 to August 2019). Results: The median age diagnosis was 16.9 (range 10.1-18.5) years, 80% were female. manifestations galactorrhea (10/20, 50%) amenorrhea (9/20, 45%) among females visual field defects (3/5, 60%) headaches (2/5, 40%) males. our cohort, macroadenomas accounted for 56% cases, rate overall responsiveness dopamine agonists (DAs) (10/18). Male gender, prolactin (PRL) level diagnosis, presence panhypopituitarism positively correlated maximum tumor diameter (r = 0.443, P 0.026; r 0.710, < 0.001; 0.623, 0.001, respectively). After trans-sphenoidal approach (TSA), 53% (8/15) showed normalization PRL level. Three patients, underwent gamma knife surgery (GKS) owing either resistance or intolerance DAs recurrence after TSA, achieved normal accompanied marked reduction symptom remission. Conclusions: macroprolactinoma more prevalent than microprolactinoma adults. increased levels, are closely related macroprolactinomas adolescents.
