Émergence de la résistance à la colistine chez les entérobactéries : une brèche dans le dernier rempart contre la pan-résistance !

摘要: Resume La lutte contre les bacteries multi-resistantes aux antibiotiques (BMR) a Gram negatif represente un enjeu majeur de sante publique, particulierement illustree par la dissemination des enterobacteries productrices carbapenemases (EPC) ne restant sensibles le plus souvent uniquement colistine. Contrairement medecine veterinaire, ou colistine est largement utilisee dans traitement d’infections colibacillaires filieres animales production, en humaine, elle longtemps ete ecartee protocoles therapeutiques raison sa toxicite, particulier renale. Elle redevenue antibiotique prescrit pour humaines severes liees resistantes toutes autres options avec l’emergence EPC notamment. Dans pays prevalence importante, tels que Grece l’Italie, resistance constante augmentation. Cette l’essentiel liee mutations chromosomiques divers genes conduisant modifications charge du LPS, ce qui va empecher fixation Recemment, plasmidiques colistine, mcr-1 et mcr-2 ont decrits chez nombreuses especes d’enterobacteries (Salmonella, Escherichia coli…) isolees Asie Sud-est, Amerique Sud, Afrique, au Danemark, Royaume-Uni, Portugal, mais aussi France, surtout l’animal et une moindre mesure l’homme. L’emergence plasmidique sont extremement inquietantes, compromettant dernier rempart sur route vers toto-resistance antibiotiques, d’autant pipeline perspectives nouvelles molecules infections assez limites.
