摘要: • Scleredema associated with a monoclonal gammopathy and generalized skin pigmentation is described in 56-year-old man hyperlipoproteinemia cardiovascular disease. The patient had IgG-λ paraproteinemia, without any evidence of multiple myeloma or immunoglobulin deposition affected skin. Ultrastructural studies pigmented lesional showed increased transfer melanosomes to basal keratinocytes dermal melanophages containing complex melanosomes. In addition, cytoplasmic, electron-opaque lipid droplets were seen approximately every third keratinocyte melanocyte, while only an occasional cell contained droplets. hyperpigmentation appeared be directly related the scleredema, was likely consequence hyperlipoproteinemia. findings further support contention that paraproteinemia may, some patients, features scleredema adultorum.
