摘要: Abstract : Spurred by a controversial report from the Institute of Medicine on prevalence medical error, To Err Is Human, profession has seen an increase in event reporting systems at international, national, and institutional levels. Aviation, with its long history systems, shown that these can yield previously unknown, but safety-critical information for developing proactive approach to managing human error. Despite many similarities between health care aviation, have not been well received care. Studies physicians are reluctant participate programs errors, underreporting adverse events may be as high 96 percent. These findings suggest success system is determined attitudes perceptions frontline providers. Therefore, prior implementing system, assessment opinions providers should conducted identify critical barriers reporting. The University Texas Human Factors Research Project developed survey instrument designed assess wide array deemed relevant implementation systems. This paper summarizes preliminary recommendations successful system.
