Clinical Features and Outcomes of Endoscopic Treatment for Stones in Stemware-Shaped Common Bile Ducts: A Multicenter Data Analysis

摘要: Background/Aims: Various anatomical features of the biliary tree affect ability to remove difficult common bile duct (CBD) stones. In this study, we evaluated clinical characteris- tics and outcomes endoscopic treatment stones in stemware-shaped CBDs. Methods: Thirty-four patients with a stone CBD who were treated at dif- ferent tertiary referral centers from January 2008 Decem- ber 2012 studied retrospectively. When removal failed, percutaneous or direct peroral cholangioscopic litho- tripsy, retrograde drainage, surgery was performed as second-line procedure. Results: The overall success rate first-line procedure 41.2%. Five 34 (14.7%) experienced procedure-related com- plications. No mortality occurred. Mechan- ical lithotripsy required completely 13 (38.2%). Conversion 20 (58.8%). Mechanical needed 75% 66.7% those size <1 cm ≥1 cm, respectively. Stone recurrence occurred two (9.1%) after 6 months 27 months, respec- tively. Conclusions: is challenging. careful assess- ment before procedures. (Gut Liver 2015;9:800-804)
