Safety considerations for magnetic resonance imaging of pacemaker and ICD patients

摘要: Magnetic resonance imaging (MRI) has evolved as an important diagnostic tool in medicine. However, due to safety concerns the presence of an implanted cardiac pacemaker is considered be a contraindication MRI most medical centers. Previous vitro and vivo studies showed that strong electromagnetic fields environment may cause asynchronous pacing, inhibition demand pacemaker, induced currents system pacing heart, or potential risk thermal myocardial injury around pacing lead tip. ICDs addition are not able charge apply a therapy strong magnetic field. There reported lethal consequences patients with pacemakers. there are also recent reports small series patients who have safely undergone MRI. At present time, non-MRI modalities should considered, whenever possible, make a diagnosis recipients. If other modalities are adequate, at low field strengths (0.5 Tesla or less) with careful monitoring preparation for adverse events may be considered only experienced centers. The device be programmed asynchronous, if possible non-pacing mode, during Pacemaker mode changes, inappropriate pacing behavior induced voltages, localized heating the lead tip primary issues that remain fully investigated future studies.
