摘要: Background.— Migraine is a highly prevalent chronic disorder associated with significant morbidity. Chronic daily headache syndromes, while less common, are likely to be recognized, and impair quality of life an even greater extent than episodic migraine. A variety screening diagnostic tools for migraine have been proposed studied. Few investigators developed evaluated computerized programs diagnose headache. Objectives.— To develop determine the accuracy utility assessment tool (CHAT). CHAT was designed identify all major primary disorders, distinguish from types, recognize medication overuse. Methods.— using expert systems approach diagnosis, initial branch points determined by frequency duration. Appropriate clinical criteria presented relevant brief longer-lasting headaches. posted on web site Microsoft active server pages SQL-server database server. convenience sample patients who adult urgent care department headache, in family practice waiting room, were solicited participate. Those completed on-line questionnaire contacted interview. Results.— One hundred thirty-five 117 interview. correctly identified 35/35 (100%) 42/49 (85.7%) transformed also 11/11 tension-type 2/2 1/1 cluster headache. Medication overuse recognized 43/52 (82.7%). The most common misdiagnoses seen or new persistent headache. Fifty referred their physician 62 clinic. Of 29 PCP confirmed diagnosis migraine, 25 made follow-up appointment, diagnosed 19, initiated migraine-specific therapy prophylaxis 17. Conclusion.— described system displays high other including syndromes overuse. As part disease management program, led receiving appropriate diagnoses therapy. Limitations include patient willingness utilize introducing such process into culture medical care, difficult distinction
