摘要: Alzheimer’s disease (AD) has been identified as one of the 25 top causes years lived with disability. Currently, no pharmacological treatment can prevent, slow down, or stop course this disease. From clinical and health management perspectives, Mild Cognitive Impairment – a condition representing risk factor for development dementia - early stages AD are most interesting conditions interventions aimed at delaying further decline. Telemonitoring telerehabilitation home-based services have advocated to provide manifold benefits people cognitive disorders. In paper, we will describe concept vision enlightening Ability, technology-enhanced continuity-of-care home program After describing platform architecture, present use case showing how it disorders both formal informal caregivers by generating intertwining support in process care, enhancing well-being, conditions, inclusion.
