The traditional belief in cardiac surgery was that the damage perpetrated by an embolus caused occlusion of arterial branch, resulting ischemic event subsequent infarction. However, ongoing research has demonstrated mere passage a deformable (air, lipid, or semi-solid clot) will disrupt endothelium as it is extruded through vessel. A cascade events follows endothelial irritation. In closed environment brain, disruption blood-brain barrier been after lipid microemboli. breakdown causes marked brain swelling, increased intracranial hypertension, possible increase size lesions associated with larger occlusive emboli.
