Abstract: Background: To assess the diagnostic value of magnetic resonance (MRC) when evaluating complications a large series transplants. Methods: One hundred twenty-nine patients prospectively underwent (MR) imaging at 1.5-T device after transplantation (OLT). After preliminary acquisition axial T1- T2-weighted images, MRC involved respiratory-triggered, thin-slab (2 mm), heavily fast spin-echo breath-hold, thick-slab (10–50 mm), single-shot sequences. images were blindly evaluated by two experienced readers conference to determine anatomy presence complications, whose final diagnosis was based on endoscopic retrograde cholangiography, percutaneous trans-hepatic integrating clinical follow-up with ultrasound and/or findings. Results: Biliary found 60 (46.5%) represented ischemic-type lesions (n = 21); anastomotic strictures (n = 13); non-anastomotic (n = 5); associated lithiasis (n = 6); papillary dysfunctions (n = 9). The sensitivity, specificity, positive predictive value, negative reviewers for detection all types OLT 98%, 94%, respectively. Conclusions: is reliable technique post-OLT should be recommended before planning therapeutic interventions.
