Introducing standardized "readbacks" to improve patient safety in surgery: a prospective survey in 92 providers at a public safety-net hospital

摘要: Communication breakdowns represent the main root cause of preventable complications which lead to harm surgical patients. Standardized readbacks have been successfully implemented as a pillar professional aviation safety for decades, ensure safe closed-loop communication between air traffic control and individual pilots. The present study was designed determine perception staff in perioperative services regarding role standardized improving patient surgery at single public safety-net hospital level 1 trauma center. A 12-item questionnaire sent 180 providers Denver Health Medical Center. survey participants’ (1) appropriateness current readback processes; (2) willingness attend future training module on this topic; (3) specific scenarios may be effective; (4) perceived major barriers implementation readbacks. Survey results were compared departments (surgery versus anesthesia) roles (attending or midlevel provider, resident physician, nursing staff), using non-parametric tests. response rate 50.1 % (n = 92). Respondents overwhelmingly recognized reducing errors safety. There strong agreement among respondents support participation program. no difference responses anesthesia departments. statistically significant healthcare short (p < 0.001). Resident physicians less likely endorse importance (p = 0.01) willing (p < 0.001), staff. challenge respondents, emanated from their responses, appeared relate determining ideal most appropriately used. Overall, strongly felt that had an important handoffs, orders critical results, counting verifying instruments, delegating multiple tasks. majority all appear perceive effective tool and/or preventing adverse events care patients, derived breakdown caregivers. Further work needs done define exact clinical efficiently implemented, including definition uniform set scripted quotes phrases, should concert with model.
