摘要: To apply duodenum-preserving pancreatic head resection (DPPHR) as radical procedure for benign or low-grade malignant tumors, it needs the reconciliation of complete and preservation bile duct peripancreatic vessels. Several modifications have been introduced applied to remove these lesions, however, techniques not made clear in management vessels duct. The long-term outcomes DPPHR reported extremely rare comparison with pylorus preserving pancreatoduodenectomy (PPPD) tumors. angiograms by multi-detector row CT (MD-CT) can be reconstructed more physiologically than selective angiography. anterior arcade is predominant 43% 64 patients. Therefore, we modified include a both posterior arterial arcades. covered parenchyma various ways. are also introduced. We performed 21 DPPHRs 19 PPPDs patients tumor. There was no significant difference operative factors. postoperative death one patient PPPD, but none DPPHR. complications PPPD were often that recurrence follow-up period from 2 216 months. Both exocrine endocrine function results following superior those PPPD. should favored over tumors pancreas if there compromise oncologic radicality.
