Lymphocytic intracellular calcium in a patient with complicated verapamil overdose.

摘要: Overdose with calcium channel blockers (CCBs) may lead to serious complications. CCBs act by blocking entry into the cell, thus lowering intracellular ([Ca2+]i). [Ca2+]i during CCB overdose has not yet been reported. We measured in lymphocytes of a patient acute verapamil complex clinical picture. A 59-year-old woman was admitted after suicidal ingestion 7200 mg sustained-release preparation. She presented hypotension, complete atrioventricular block, stupor, hypokalemia, and hyperglycemia. Acute oliguric renal failure, pancreatitis, adult respiratory distress syndrome further complicated her medical course. Treatment supportive she recovered completely. Intracellular ([Ca2+]i) patient's using spectrofluorometer calcium-sensitive dye Fura-2-acetoxymethyl ester. Thirty nine hours ingestion, low at 52 nM (compared 80 healthy control subject). Lymphocytic did respond stimulation phytohemagglutinin (PHA). Fourteen days overdose, had completely, lymphocytic still 55 nM. At this time, there an incomplete response PHA lymphocytes. Three months normal, normal PHA. Verapamil run course, but full recovery is be hoped for care. Cellular intoxication, as reflected [Ca2+]i, prolonged lags behind weeks.
