Les formes graves de la grippe A(H1N1) 2009 chez la femme enceinte: expérience du centre hospitalier universitaire de Fès, Maroc et revue de la littérature

摘要: INTRODUCTION: Le but de cette etude est decrire les caracteristiques epidemiologiques, cliniques, paracliniques ainsi que l’evolution des femmes enceintes ou en post partum atteintes formes graves Grippe A(H1N1) 2009. METHODES: C’est une prospective observationnelle monocentrique, menee au sein notre service reanimation mere et enfant centre hospitalier universitaire Hassan II a Fes, sur periode 3 mois, allant novembre 2009 janvier 2010. RESULTATS: L’âge moyen etait 28 ans, dans 85% cas la grossesse se situaient troisieme trimestre, le syndrome grippal constant, SpO2 initiale moyenne 86%. A radiographie thoracique, un alveolaire bilateral toujours present. L’infection virale confirmee tous par polymerase chain reaction. Chez patientes PaO2/FiO2 inferieure 300. L’Oseltamivir l’antiviral utilise chez toutes parturientes. Un detresse respiratoire aigu ete developpe 28% parturientes, elles ont ventilees artificiellement avec niveaux pressions expiratoires positives 14 +/- 1 cmH2O. L’evolution favorable 71% cas, cependant, 2 deces deplores. CONCLUSION: Les resultats rejoignent donnees litterature, savoir, risque accru pour femme enceinte developper forme grave, presentation clinique similaire reste population, l’interet vaccination d’un traitement antiviral precoce role l’ECMO hypoxemies refractaires.
