Influence of sepsis on higher daily dose of amikacin pharmacokinetics in critically ill patients

摘要: BACKGROUND AND OBJECTIVES: Severe sepsis is a major problem as cause of high rates morbidity and mortality in intensive care units (ICU). Aminoglycosides are an impor- tant group antimicrobials used for severe sep- sis. However, aminoglycoside pharmacokinetics ICU patients may be altered during sepsis, which can affect the drug concentrations. There- fore, this study was undertaken to examine relationship between amikacin disposition kinet- ics after 25 mg/kg loading dose hemody- namic response well clinical pa- rameters, population critically ill patients. METHODS: In work, 30 who were candidate therapy following Gram negative enrolled. The pharmacoki- netic profile by non-compartmental model calculated each patient. RESULTS: Mean volume distribution 0.36 ± 0.07 L/kg mean serum clear- ance 3.88 0.97 ml/min/kg. case Vd, APACHE II score correlation significant. clearance, two covari- ates including creatinine clearance Sr Cr significant found. CONCLUSIONS: It appears necessary use higher dosage (≥ mg/kg) considering hemodynamic sepsis. To achieve therapeutic concentration close monitoring shift from toward value more representative patient subpopulation crucial.
