Course of functional status and pain in osteoarthritis of the hip or knee: a systematic review of the literature.

摘要: Objective. To systematically review studies describing the course of functioning in patients with osteoarthritis (OA) hip or knee and identifying potential prognostic factors. Methods. A systematic search was performed. Studies involving OA, >6 months followup, outcome measures on functional status pain were included. Methodologic quality assessed using a standardized set 11 criteria; qualitative data analysis Results. Approximately 6,500 titles abstracts screened 48 publications considered for inclusion. Eighteen studies, 4 which met high methodologic criteria, For there limited evidence that do not change during first 3 years followup. After years, however, worsening seen. conflicting after years. Furthermore, established negative associations between future laxity, proprioceptive inaccuracy, age, body mass index, intensity. In contrast, greater muscle strength, better mental health, self-efficacy, social support, more aerobic exercise protective factors Conclusion. Pain OA seem to deteriorate slowly, specific subgroups, prognosis followup either worse better, as both risk identified. Prognostic included biomechanical factors, psychological clinical treatment modalities. strengthen evidence, further high-quality longitudinal research is needed.
