摘要: OBJECTIVE There are many classification systems for injuries of the thoracolumbar spine. The recent Thoracolumbar Injury Classification and Severity Score (TLICS) has been shown to be a reliable tool adult patients. aim this study was assess reliability TLICS system in pediatric validity is assessed companion paper. METHODS medical records patients with acute, traumatic fractures at single Level 1 trauma center were retrospectively reviewed. A calculated each patient using CT MRI, along neurological examination recorded patient's record. TLICSs compared type treatment received. Five raters scored all separately interrater reliability. RESULTS calculations completed 81 mean age 10.9 years. Girls represented 51.8% population, 80% white. most common mechanisms injury motor vehicle accidents (60.5%), falls (17.3%), all-terrain (8.6%). 3.7 ± 2.8. Surgery choice 33.3% agreement between TLICS-suggested actual received statistically significant (p < 0.0001). ranged from moderate very good, Fleiss' generalized kappa (κ) value 0.69 suggestion among patients; however, decreased when MRI used contribute TLICS. κ 0.73 0.57 only vs CT/MRI or only, respectively Furthermore, suggested worse as part assessment. CONCLUSIONS demonstrates good physicians assessing fracture Physicians should cautious aid surgical decision-making process.
