Primary angiitis or PACNS, disease once considered extremely rare, recently has been reported more frequently. Traditional views PACNS held it to be relentless and uniformly fatal if untreated, but is now viewed as heterogeneous, with some relatively benign subsets. Its pathophysiology understood.
