Coronary bypass graft fate. Angiographic study of 1,179 vein grafts early, one year, and five years after operation.

摘要: A total of 1,179 vein grafts were studied angiographically in 353 (45%) unselected survivors (male, mean age 45.5 years) 786 coronary bypass operations. Studies conducted early (0.96 months), 1 year (12.8 and 5 years (59.7 months) postoperatively. previously described technique was used to grade the patency grafts, a new assess intimal irregularity, presumably caused by atherosclerosis; this indicated both surface distribution disease profile (relief or elevation). Ten percent, 17%, 26% occluded early, at year, years, respectively. Distal anastomotic defects commonest cause for low grades classification. Irregularities patent increased from 9% 42% with 11% all lesions 20% having high (more than 50% graft stenosis); categorized as showing widest spread, 17% relief 34% years. Thus, we believed be atherosclerotic proliferated spread elevation. All severely diseased study had been normal outline early; 79% free year. newly 82% good 78% 77% patency. Normal appearance intima no prognostic value findings. However, 62% showed deterioration 28% occluded. The differences between these outcomes are highly significant (p less 0.0005). In conclusion, irregularity compatible atherosclerosis after operation carried poor prognosis adverse angiographic change On other hand, normally appearing predictive valve despite generally better nondiseased grafts.
