摘要: Ipswich Hospital Emergency Department played a vital role in the Post AcuteTreatment Home Program (PATH) of West Moreton District Health Service.PATH used two strategies to reduce district reliance on acute hospital beds: short-stayunit for rapid assessment, treatment and early discharge patients with simpleconditions; hospital-in-the-home program utilising community health servicesto treat conditions.The enhanced existing services create new stream acutepatients promote cultural shift from fragmented care responsibilityfor total episode patient care.
