Prospective assessment of primary rectal cancer response to preoperative radiation and chemotherapy using 18-fluorodeoxyglucose positron emission tomography.

摘要: PURPOSE: The purpose of this prospective study was to determine the ability fluorine-18 fluorodeoxyglucose positron emission tomography assess extent pathologically confirmed rectal cancer response preoperative radiation and 5-fluorouracil-based chemotherapy. METHODS: Patients with primary deemed eligible for chemotherapy because a clinically bulky or tethered tumor endorectal ultrasound evidence T3 and/or N1 were prospectively enrolled. Positron CT scans obtained before (5,040 cGy pelvis 2 cycles bolus 5-fluorouracil leucovorin) repeated four five weeks after completion In addition routine pathologic staging, detailed assessment performed independently by two pathologists. parameters studied included conventional measures such as standardized uptake value (average maximum), tomography-derived volume (size), novel parameters: visual score change in total lesion glycolysis. RESULTS: Of 21 patients enrolled, data (pretreatment posttreatment tomography, complete assessment) available on 15 patients. All demonstrated This 100 percent cases compared 78 (7/9) CT. addition, one parameter (visual score) accurately estimated 60 (9/15) 22 (2/9) CONCLUSIONS: pilot demonstrates that imaging adds incremental information cancer. However, further studies larger series are needed verify these findings strategy aimed at identifying suitable sphincter-preserving surgery.
