The aim of this study was to investigate whether the analysis MMSE pentagon copy could be useful diagnostic screening tool in prodromal DLB. Methods Fifty-three patients Mild Cognitive Impairment (MCI) were followed over 3-years until diagnosis DLB (MCI–DLB: n = 30) and AD (MCI–AD: n = 23) made according standard criteria. At first assessment underwent thorough including Qualitative Scoring Pentagon Test (QSPT), NPI, UPDRS as well questionnaires determine fluctuations sleep disorders. Results percentage subjects who unable correct number angles test 45.1% MCI–DLB 8.3% MCI–AD (sensitivity 41.1%; specificity 91%). Attentive/executive functions visual-spatial abilities worse group, while episodic memory greater MCI–AD. Subtle extrapyramidal signs (63%) RBD symptoms (56%) most frequent clinical features supporting MCI–DLB. Conclusions We suggest that poor performance determining when copying test, together presence subtle serve predictive for early
