Brachial plexus dose tolerance in head and neck cancer patients treated with sequential intensity modulated radiation therapy.

摘要: We aimed to study the radiation induced brachial plexopathy in patients with head and neck squamous cell carcinoma (HNSCC) treated Sequential Intensity Modulated Radiation Therapy (S-IMRT). This IRB approved included 68 HNSCC consecutively. Detailed dose volume histogram data was generated for ipsilateral contralateral plexus (BP) volumes receiving a specified (Vds) i.e. V50-V75 Gray covering percent of BP (Dvs) D5-D30 maximum point doses (Dmax). To assess injury all patients’ charts were reviewed detail sign symptoms damage. Post-hoc comparisons done using Tukey-Kramer method account multiple significance testing. The mean significantly different (p < .05) based on tumor site, nodal status stage. V50, V60, V70, V75 7.01 cc, 4.37 cc, 1.47 cc 0.24 cc, respectively. delivered ≤5% 68.70 Gy (median 69.5Gy). None had acute or late any other significant neurological complications, minimum follow up two years (mean 54 months). In this cohort, at two-years up, 68.7Gy, median 69.5Gy BP, Dmax 72.96Gy did not result when S-IMRT technique. However, longer is needed.
