Closed-loop glycemic control with a wearable artificial endocrine pancreas: variations in daily insulin requirements to glycemic response

摘要: We succeeded in miniaturizing a needle-type glucose monitoring system with characteristics suitable for application wearable, closed-loop control system. A wearable artificial endocrine pancreas (12 × 15 6 cm, 400 g) consisting of sensor, microcomputer that calculates insulin and glucagon infusion rates, two roller pumps was developed. Continuous by sensor inserted the subcutaneous tissue forearm or abdomen healthy diabetic volunteers revealed concentrations were 10% lower than, but highly correlated with, blood range 49–388 mg/dl. Glycemic established patients intravenously infusing response to measured on moment-to-moment basis period several days. By comparing glycemic obtained each patient treated multiple injections open-loop infusion, superiority feedback clearly demonstrated. During continuous regulation, day-to-day variations requirements recognized both basal postprandial rates identical meals exercise. These data suggest feasibility long-term subjects pancreas, indicate overcome changes individual metabolic basis, may be essential ambulatory patients.
