Selective Nonoperative Management of Penetrating Abdominal Injuries in Children

摘要: Background: Nonoperative management of solid organ injuries caused by blunt abdominal trauma has been the standard care for many years. However, operative is considered penetrating most surgeons. The aim this study was to assess feasibility selective nonoperative in children. Methods: A total 90 children suffering from between 2003 and 2008 were evaluated. Patients who had hemodynamic instability or signs bowel perforation underwent an immediate laparotomy. remaining patients observed with serial clinical examinations, radiologic evaluation, hemoglobin level. Results: There 76 boys 14 girls. mean age 9.9 years (range, 1-16 years). mechanism injury stab wound 60 (67%) gunshot 30 (33%). commonly injured (51.7%). Omentum eviscerated through seven patients; none these injury. Although 51 (56.6%) treated nonoperatively, 39 (43.4%) required surgical treatment (19 wound, 20 gunshot). Of surgery, 6 (15.3%) found have no significant during surgery. all, initially followed two complications. Conclusion: majority wounds can be managed nonoperatively children, when there hollow viscus perforation.
