摘要: It has been proven in experimental and clinical studies that combined perioperative systemic antibiotic prophylaxis prolonged nonresorbable topical antibiotics directed against common intestinal Gram-negative Gram-positive pathogens a mechanically cleaned bowel are effective preventing anastomotic leak (AL). For 60 years, evidence accumulating AL is caused by microbial pathogenicity. Examples E. faecalis P. aeruginosa, which develop the ability to breakdown collagen and/or cleave host Matrix metalloproteinase 9 (MMP9). The surgical trauma seems trigger such complex reactions as gentotypical phenotypical changes commensal microbiota, turning them into tissue-destroying, leak-inducing pathogens. Investigations of further molecular mechanisms ongoing. use disrupts endogenous microbiome causes resistance. therefore important find therapies leave intact target virulence expression. One approach avoid indiscriminate elimination all potential with simultaneous damage protective microbiota represented phosphate-loaded polyethylene glycopolymers.
