Sequence of retrograde atrial activation in patients with dual atrioventricular nodal pathways.

摘要: To characterize the sequence of retrograde atrial activation in presence dual atrioventricular (AV) nodal pathways, we analyzed electrophysiologic data from seven patients whom discontinuous AV and ventriculoatrial conduction curves could be induced with programmed electrical stimulation. In all patients, electrograms high right atrium (HRA), lateral (LRA), low septal (SRA) proximal coronary sinus (PCS) near ostium were simultaneously recorded at a paper speed 150-250 mm/sec. During ventricular extrastimulation incremental pacing, via fast pathway resulted SRA before PCS, HRA LRA activation. However, abruptly changed shift to slow conduction. Characteristically, during pathway, PCS preceded by 5-20 msec was accompanied an alteration temporal relationship between patients. These observations suggest that anatomically, common is broad area permits have exit located posteriorly, inferiorly left tachyarrhythmias should determined caution attempting differentiate normal anomalous bypass tract
