摘要: Abstract PURPOSE: To report treatment of persistent corneal epithelial defects unresponsive to other therapies by extended wear a fluid-ventilated gas-permeable scleral contact lens. METHODS: In this retrospective study, 14 eyes 13 consecutive patients referred for the that failed heal with conventional or developed after penetrating keratoplasty were fitted an extended-wear These included seven six Stevens-Johnson syndrome and who did not have syndrome. Twelve had undergone recent keratoplasty. All lens designed avoid intrusion air bubbles under its optic. An antibiotic corticosteroid added fluid reservoir instilled before each insertion in 12 eyes. The lenses worn continuously except brief periods removal purposes cleaning, replacement reservoir, examination photography cornea. RESULTS: Five associated healed. four these re-epithelialized within 7 days, fifth healed 27 days wear. A sixth defect initially subsequent seventh eye 3 wear, but subsequently recurred. Three non–Stevens-Johnson 36 hours, 6 respectively. Of (six 14) one multiple amniotic membrane grafts. Microbial keratitis occurred (four graft failure eye, all which required repeat CONCLUSION: Extended appropriately was effective promoting healing some therapeutic measures. Re-epithelialization appears be aided combination oxygenation, moisture, protection fragile epithelium afforded However, microbial represents significant risk.
