Cost effectiveness of an intensive blood glucose control policy in patients with type 2 diabetes: economic analysis alongside randomised controlled trial (UKPDS 41)

摘要: Abstract Objective: To estimate the cost effectiveness of conventional versus intensive blood glucose control in patients with type 2 diabetes. Design: Incremental analysis alongside randomised controlled trial. Setting: 23 UK hospital clinic based study centres. Participants: 3867 newly diagnosed diabetes (mean age 53 years). Interventions: Conventional (primarily diet) policy a sulphonylurea or insulin. Main outcome measures: per event-free year gained within trial period. Results: Intensive increased treatment costs by £695 (95% confidence interval £555 to £836) patient but reduced complications £957 (£233 £1681) compared management. If standard practice visit patterns were assumed rather than conditions, incremental management was £478 (-£275 £1232) patient. The time group 0.60 (0.12 1.10) years and lifetime gain 1.14 (0.69 1.61) years. £1166 (costs effects discounted at 6% year) £563 not discounted). Conclusions: significantly substantially free complications.
