An algorithm combining age, total prostate-specific antigen (PSA), and percent free PSA to predict prostate cancer: Results on 4298 cases

摘要: Abstract Objectives. To (1) determine if patient age and total prostate-specific antigen (PSA) levels could enhance the ability of percent free PSA to distinguish prostate cancer from benign disease within 4.0 20 ng/mL range; (2) define probability based on age, PSA, PSA; (3) a cutoff that distinguishes malignant disease. Methods. The 3773 urologically referred patients with serum values between had sextant biopsy diagnosed as either prostatic carcinoma (1234) or (2539) 60 days specimen collection. We created logistic regression model, using assign cancer, tested model an additional data set (525 patients) calculate sensitivity specificity. Results. An 18% detected 95% biopsies identified 34% negative in validation set. This approach yielded 11% percentage point increase specificity over alone. A 20% 90% cases 42% biopsies. Conclusions. is more effective than alone differentiating cancer. may assist physicians regarding need for biopsy.
